Lie on your side and the upper leg falls forward under its own weight. The pelvis rotates to follow it, the lower back twists to match, and the whole arrangement is held there for hours — which is why side sleepers so often get up stiff through the hips and lower back.

Why it is notched instead of square
A plain cushion between the knees is better than nothing, right up until you move. This one is cut with a deep notch in its base: the lower knee settles down into the notch and the upper knee rests on the peak, so the shape holds its own position instead of relying on you clamping your legs together to keep it there.
The strap is the part most knee pillows leave off
A flat elasticated band is sewn across one face. It goes over your top thigh, just above the knee. Will this fix my back or hip pain?
It addresses one specific mechanical thing: the top leg dropping forward and taking the pelvis with it while you sleep on your side. Plenty of people find that helps them. It is a foam pillow, not a treatment. Pain that persists or worsens, or comes with numbness or weakness, is worth taking to a clinician rather than to a cushion.
How firm is it?
Memory foam, so it gives under the knee and then holds. Our own reviewers are genuinely split on it — some say it takes a week to settle in, at least one found it softer than she wanted and would have preferred her top leg to sink in less. If you specifically want a rigid, unyielding block, this is not that.
Where exactly should it sit?
Between the knees, with the notch in the base cradling the lower knee and the strap over the top thigh. Some people prefer it a little lower, closer to the shins. Try both — the right position is the one you stop noticing.
Will it stay put if I move a lot?
Better than a flat cushion will, because of the notch and the strap. If you turn fully from one side to the other you will generally take it with you. Very restless sleepers may still reposition it occasionally, and a few reviewers say it has worked loose on them once or twice in a night.
Is it big enough? Is it too big?

Can I use it during pregnancy?

Is it any use for back sleepers?

How do I keep it clean?
Unzip the cover, take it off and wash the cover. Keep the foam core itself out of the water — spot-clean it with a barely damp cloth if it needs it and let it dry completely. Check the care label on your cover before choosing a wash temperature.
